Policies & Procedures

Please read and initial the policies below. Your signature at the end of the form indicates that you accept these policies.

(Initial) Hill Country Animal Hospital requires that all boarding pets be current by the hospital’s standards on all vaccinations. Hill Country Animal Hospital will update any vaccine and/or parasite screen, if my pet is not current. It is my responsibility to provide the hospital with a copy of my pet’s most recent vaccination history, if the hospital is not my normal veterinarian. I will be financially responsible for any vaccinations given to my pet that are required for boarding.

DOGS: All dogs are required to be current on their Rabies, DHLPP, Bivalent Influenza, Bordetella, and Intestinal Parasite Screen. Hill Country Animal Hospital does require the Bordetella biannually. Although the Heartworm Test is not required for boarding, it is highly recommended.

CATS: All cats are required to be current on their Rabies and FVRCP. Although the Leukemia vaccine and Intestinal Parasite Exam are not required for boarding, they are highly recommended.

(Initial) Hill Country Animal Hospital requires that all pets be free of external parasites (fleas and ticks) when admitted for boarding. If external parasites are found on my pet, they will be treated upon admission at the owner’s expense.

(Initial) Hill Country Animal Hospital allows treats for my pet, however, no toys are permitted to be left with my pet, as they can be choking hazards.

(Initial) Hill Country Animal Hospital will provide all the bedding needed for my pet. Hill Country Animal Hospital does not accept personal bedding and is not responsible for any items or personal belongings that are lost or damaged.

(Initial) I have read and understand the policy of boarding charges. Should I pick up after 2:00pm, I will incur an additional boarding charge for that day unless I have requested a bath or groom, in which case I would be charged accordingly. I also understand that if I choose to patriciate in the courtesy pick up on Sundays, I will be charged for that day.

(Initial) All fees for boarding and all related services are due upon pickup.

Owner Release: I certify that I have read the policies and procedures listed above. The hospital and staff will NOT be held liable for any problems that develop, provided reasonable care and precautions are followed. I understand that ANY problem that develops with my pet in my absence will be treated as deemed best by the veterinarian and I assume full responsibility for the treatment expense involved.
